The term impotence (ED) refers to the inability to achieve or maintain an erection suitable for sexual intercourse. The main causes of erectile dysfunction include diabetes, cardiovascular conditions, hormonal imbalance and neurological problems (for example, stemming from a prostatectomy). It is important to note that organic ED is not a disease by itself but rather a symptom of another existing condition. More rarely, they symptoms are psychogenic. Work-up

Listed below is the erectile dysfunction work-up, which will help specialists determine the optimal treatment.

Patients experiencing ED symptoms will be referred for the following investigations:

  • Extensive blood work-up (CBC, CMP, glucose levels, lipid profile, hormonal profile); patients over 40 will also have their PSA levels checked
  • US of urinary tract with post-void residual urine test
  • Urologic andrologist consultation

Depending on the results, you may be referred to one or more of the following additional tests:

  • Nocturnal penile tumescence (NPT) test to determine where the symptoms are organic or psychogenic
  • Penile Doppler ultrasound to evaluate the condition of penile blood vessels and cavernous bodies, as well as blood flow velocity
  • Intracavernous injection test to evaluate penile blood vessel function; this test may also help determine where the symptoms are organic or psychogenic
  • TRUS
  • STD urine test
  • Lumbar MRI scan

Following the aforementioned investigations your treating urologist may (if indicated) refer you to a sexopathology expert, neurologist, cardiologist and/or a psychologist.
